Make sure your patient qualifies for a no-cost blood pressure cuff under TennCare Medicaid’s Policy and is covered through Wellpoint Tennessee. You can find a list of eligible codes in the FAQs below.
Complete the order form
You’ll be asked to provide details including your patient’s name, address, and clinical diagnosis.
Let your patient know their blood pressure cuff is on the way!
Once your order is confirmed, your patient will receive their blood pressure cuff within 10 days.
